Referring your patient to Dr Simon Fleming, orthopaedic hand & wrist surgeon

Dr Simon Fleming is now accepting new patients in private practice at Hand and Nerve Day Surgery Centre, Rockhampton QLD. 

With an emphasis on a holistic, multidisciplinary approach, Dr Fleming offers specialist management of all hand and wrist conditions, including:

• Cubital tunnel syndrome
• Carpal tunnel syndrome
• De Quervain’s tenosynovitis 
• Dupuytren’s contracture
• Stenosing tenosynovitis (trigger finger)
• Fractures of the hand and wrist
• Nerve, tendon, and ligament injuries of the hand and wrist
• Ganglion cysts
• Rheumatoid arthritis of the hand and wrist
• Osteoarthritis of the hand and wrist
• Carpometacarpal joint arthritis (base of the thumb arthritis)

To include in your referral

A short, focused referral is more helpful than a long one. Where possible, please include:

• The clinical question – What you would like the surgeon to address, such as diagnosis, consideration of surgery, a second opinion, or chronic pain assessment.
• Presenting complaint – Include the side affected, mechanism of injury, duration of symptoms, aggravating and relieving factors, and, for upper limb conditions, the patient’s hand dominance and occupation.
• Examination findings – Summarise the relevant clinical findings, including range of motion, instability, neurological assessment, special tests performed, and any relevant clinical photographs.
• Investigations to date – Include previous imaging and reports, noting the imaging provider and date where possible.
• Past history and medications – Include any relevant medical history, current medications (particularly anticoagulants and immunosuppressants), diabetic control, and smoking status.
• Funding pathway – Advise whether the patient is privately insured, self-funded, covered by a DVA Gold or White Card, or has a WorkCover claim, so the appropriate consultation pathway can be arranged.
